Dr. Borchelt and our registered dietitians see patients individually in our new clinic for a variety of medical diagnoses, including but not limited to: diabetes, pre-diabetes, high blood pressure, elevated cholesterol, and weight management.
Our Diabetes Self-Management Education/Training program is accredited by the American Association of Diabetes Educators (AADE) and utilizes the Journey for Control conversation maps to facilitate learning in a peer setting. We offer these at two different locations.
Our Diabetes Prevention Program is pending CDC Recognition and classes are guided by a trained lifestyle coach. We have locations in five different counties: George, Hancock, Harrison, Jackson, and Stone. Optifast is a medically supervised full meal replacement program. Enrollment in the program includes six months of lifestyle education classes with a peer group to build long-term skills for success.
Shared Medical Appointments
These are group visits in which individuals can receive a combination of services for a specific health issue. Services include a medical visit, in-depth education with a provider that you may not normally have access to (such as a registered dietitian, pharmacist, nurse, exercise physiologist), as well as peer support from others struggling with the same condition. These do not replace your individual physician appointments but are an additional resource for chronic disease management.
